MEDICAL ID JEWELRY FOR FAMILY CAREGIVERS COULD SAVE YOUR LOVED ONE'S LIFE

- I am a family caregiver for my (relationship)

- Caregiver's name

- Care recipient's name

- Urgent needs and instructions for the recipient

If you wish to provide more information that can fit on your ID, you can engrave "See wallet card" and write additional information on the free wallet card that will be included with your order (make sure it is accessible to emergency personnel).

Many care recipients should also wear a medical ID that provides their health conditions (such as heart disease, diabetes, Alzheimer's, stroke risk), medications they are taking (such as blood thinners) and any food, drug or insect allergies.
